Surrender of certificate.—Every agent for collection of contributions who ceases to carry on the collection of contributions as an agent shall, within forty-two days of such cessation, surrender the certificate of registration to the Medical Benefit Council in person or by registered post, and shall inform it in Form 3 of the manner in which every medical certificate held by him has been disposed of.
