Intimation of change.—Every employer of a maternity beneficiary shall intimate to the Medical Benefit Council, in Form 5 and within ten days of the change, any change in the particulars furnished in the application for registration, and the Medical Benefit Council shall amend the certificate accordingly on payment of a fee of ten rupees.
