Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance medical practitioner shall apply to the Corporation in Form 16 for a duplicate, accompanied by a fee of five hundred rupees, and the Corporation shall issue a duplicate certificate marked as such within twenty-one days of the application.
