Application for registration.—Every nominee of a member carrying on the receipt of benefits as nominee shall make an application for registration to the Controlling Authority in Form 2, within ninety days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every register of members held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
