Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a hospital shall apply to the State Executive Committee in Form D for a duplicate, accompanied by a fee of one hundred rupees, and the State Executive Committee shall issue a duplicate certificate marked as such within fifteen days of the application.
