Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the mediation service provider shall apply to the Government of Andhra Pradesh in Form B for a duplicate, accompanied by a fee of one lakh rupees, and the Government of Andhra Pradesh shall issue a duplicate certificate marked as such within ninety days of the application.
