Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the establishment employing persons with disabilities shall apply to the Chief Commissioner in Form II for a duplicate, accompanied by a fee of two hundred rupees, and the Chief Commissioner shall issue a duplicate certificate marked as such within twenty-one days of the application.
