Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a residential facility shall apply to the District Vigilance and Monitoring Committee in Form V for a duplicate, accompanied by a fee of fifty rupees, and the District Vigilance and Monitoring Committee shall issue a duplicate certificate marked as such within sixty days of the application.
