Surrender of certificate.—Every employee who ceases to carry on the receipt of wages in a covered establishment shall, within fifteen days of such cessation, surrender the certificate of registration to the Standing Committee in person or by registered post, and shall inform it in Form 6 of the manner in which every contribution held by him has been disposed of.
