Application for registration.—Every employee carrying on the receipt of wages in a covered establishment shall make an application for registration to the Standing Committee in Form 5, within twenty-one days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every benefit held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
