Rule 5, Madhya Pradesh Mines and Minerals (Development and Regulation) Act, 1957 (Appeals) Rules, 2015 [S3G-RTD]

Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the owner of a mine within a forest shall apply to the Environmental Appellate Authority in Form I for a duplicate, accompanied by a fee of five thousand rupees, and the Environmental Appellate Authority shall issue a duplicate certificate marked as such within forty-five days of the application.
