Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the audit of accounts of a company is carried on, the particulars of every statement of affairs held by the applicant and such other particulars as are specified in Form DIR-12, and shall be signed by the applicant or his authorised agent.
