Application for registration.—Every service provider carrying on the provision of services to aggrieved persons shall make an application for registration to the Committee in Form IV, within fifteen days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every report held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
