Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the consumption of energy by a designated consumer is carried on, the particulars of every tariff held by the applicant and such other particulars as are specified in Form VIII, and shall be signed by the applicant or his authorised agent.
