Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a residential facility shall apply to the District Level Committee in Form II for a duplicate, accompanied by a fee of ten rupees, and the District Level Committee shall issue a duplicate certificate marked as such within seven days of the application.
