Surrender of certificate.—Every self-employed person who ceases to carry on the carrying on of a self-employed occupation shall, within thirty days of such cessation, surrender the certificate of registration to the Employees' Insurance Court in person or by registered post, and shall inform it in Form 16 of the manner in which every contribution card held by him has been disposed of.
