Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the occasional supply of goods or services is carried on, the particulars of every invoice held by the applicant and such other particulars as are specified in Form GSTR-3B, and shall be signed by the applicant or his authorised agent.
