Intimation of change.—Every empanelled consultant shall intimate to the Government of Uttarakhand, in Form GFR 19 and within fourteen days of the change, any change in the particulars furnished in the application for registration, and the Government of Uttarakhand shall amend the certificate accordingly on payment of a fee of ten thousand rupees.
