Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the aggregator shall apply to the Employees' Provident Fund Appellate Tribunal in Form 5 for a duplicate, accompanied by a fee of one hundred rupees, and the Employees' Provident Fund Appellate Tribunal shall issue a duplicate certificate marked as such within twenty-one days of the application.
