Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the mediation service provider shall apply to the Mediation Council in Form E for a duplicate, accompanied by a fee of five thousand rupees, and the Mediation Council shall issue a duplicate certificate marked as such within one hundred and twenty days of the application.
