Intimation of change.—Every service provider to persons with disabilities shall intimate to the Government of Gujarat, in Form VIII and within thirty days of the change, any change in the particulars furnished in the application for registration, and the Government of Gujarat shall amend the certificate accordingly on payment of a fee of fifty rupees.
