Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the audit of accounts for the purposes of tax is carried on, the particulars of every permanent account number held by the applicant and such other particulars as are specified in Form 15G, and shall be signed by the applicant or his authorised agent.
