GOVERNMENT ORDER

Subject: The preparedness of a hospital for emergencies by operators of a hospital under the Uttar Pradesh Fire Prevention and Fire Safety Act, 2005, regarding.

1. In the circumstances reported by the Fire Officer, the Government of Uttar Pradesh, after careful examination, hereby orders that every operator of a hospital carrying on the preparedness of a hospital for emergencies shall furnish Form D to the Relief Commissioner within seven days.

2. The matter has been examined. Every operator of a hospital carrying on the preparedness of a hospital for emergencies shall make an application in Form D to the Relief Commissioner within seven days, accompanied by a fee of two thousand rupees.

3. It is, accordingly, clarified that a separate application shall be made in respect of each place at which the preparedness of a hospital for emergencies is undertaken, and the Relief Commissioner shall dispose of it within seven days of its receipt.

4. Difficulties, if any, in the implementation of this government order may be brought to the notice of the Revenue and Disaster Management Department, Government of Uttar Pradesh.
