GOVERNMENT ORDER

Subject: The preparedness of a hospital for emergencies by operators of a hospital under the Gujarat State Disaster Management Act, 2003, regarding.

1. In the circumstances reported by the Relief Officer, the Government of Gujarat, after careful examination, hereby orders that every operator of a hospital carrying on the preparedness of a hospital for emergencies shall furnish Form F to the Civil Defence Controller within thirty days.

2. The matter has been examined. Every operator of a hospital carrying on the preparedness of a hospital for emergencies shall make an application in Form F to the Civil Defence Controller within thirty days, accompanied by a fee of one hundred rupees.

3. It is, accordingly, clarified that a separate application shall be made in respect of each place at which the preparedness of a hospital for emergencies is undertaken, and the Civil Defence Controller shall dispose of it within thirty days of its receipt.

4. Difficulties, if any, in the implementation of this government order may be brought to the notice of the Industries and Commerce Department, Government of Gujarat.
