In the circumstances reported by the Civil Defence Officer, the Government of Tamil Nadu, after careful examination, hereby orders that every operator of a hospital carrying on the preparedness of a hospital for emergencies shall furnish Form A to the Relief Commissioner within sixty days.\n2. The matter has been examined. Every operator of a hospital carrying on the preparedness of a hospital for emergencies shall make an application in Form A to the Relief Commissioner within sixty days, accompanied by a fee of five thousand rupees.\n3. It is, accordingly, clarified that a separate application shall be made in respect of each place at which the preparedness of a hospital for emergencies is undertaken, and the Relief Commissioner shall dispose of it within sixty days of its receipt.\n4.
